Job Description


Actalent is currently partnering with an insurance facility in TN in need of LPNs or Health Coaches to join their team. These are remote roles but candidates are required to reside within TN. Please see below for details!

Description:

  • Performing member outreach

  • Coaching members into healthy habits and gap closures. regarding coverage and provider visits

  • Assist with membership attribution

  • Assist with provider and patient engagement

  • Assist with solving for SDOH concerns

  • Provide resources to members to improve health outcome

Skills & Qualifications:

  • Bachelor’s Degree in Healthcare, Public Health, Nursing, psychology, Social Work, Health Administration, or related health field or equivalent work experience

  • LPN or Health Coach required

  • LPNs must have an active compact license for TN,MS, or AR

  • Clinical experience working in a physician’s office

  • Previous clinical outbound call outreach; motivational interviewing

  • Case Management/Utilization Management

  • HEDIS

Details:

  • Expert Level

  • 1st shift M-F

  • $23-27/hr, based on licensure

  • Only candidates local to TN will be considered
